Step 2: Provide Consent

If you are happy for your child to take part, please complete the consent form below. Participation is voluntary. You may withdraw your child at any time without giving a reason.

Step 3: Baseline Questionnaire

Once consent has been provided, please complete the short baseline questionnaire. This helps us understand your child’s recent health and experience of coughs and colds before the case study begins.

Important information

• This is an observational case study
• There are no clinic visits, tests or changes to usual medical care
• You should continue to manage your child’s health as normal and seek medical advice whenever needed
• All information provided is kept confidential and identities are kept completely anonymous
